As prescribed by the Court, the Administrator has sent letters to some claimants requesting confirmation of the reserve(s) where they lived during Long-Term Drinking Water Advisories. The First Nations affected by this requirement are:

• Clearwater River Dene Nation
• Cold Lake First Nation
• Cowichan Tribes
• Eel Ground First Nation
• Long Plain First Nation
• Peter Ballantyne Cree Nation
• Poundmaker Cree Nation
• Saddle Lake First Nation
• Samson Cree First Nation

Please respond as soon as possible. This information is necessary to process your claim and if you do not respond within 45 days of the date on your Request for Reserve Confirmation letter, your claim may be refused.

To respond, check the box or boxes next to the reserve(s) you resided on during the long-term drinking water advisory, and return it by one of the following methods:
